请假说明：
1、学生须先经家长同意再由本人填写请假条，凭该请假条才能请假。
2、学生请假只限病假和事假两种。 病假：学生因伤、病不能上课等须凭医生签发的诊断证明; 事假：在校学习期间遇特殊情况需请假的必须按规定审批。
3、学生因故不能上课者，应当事先请假获得批准。 请假一天以内，由班主任批准;请假2-3天，由班主任签署意见后，报所在年级批准。未经批准而缺席者，一律以旷课论，情节严重者给予纪律处分。
4、返校后，应及时向班主任销假。否则将作请假未归处理。
5、此条一式两联，分别作为存根(存根联：班主任)及学生进出校门的凭证(学生联：门卫室)
